New individual equipment request forms coming soon
Published: 08 May 2023EnableNSW clinicians across NSW will soon have access to new and improved fillable PDF equipment request forms (ERFs). These forms will replace the existing ERFs and will be rolled out over the next few months.
What’s different about these forms?
Each equipment category will have its own form and will be:
- more streamlined
- better aligned to the funding criteria
- easier to complete with more tick box options
What will be different about the stock form & the self-care mobility ERF?
The stock equipment request form (SERF) and the usual equipment request form for self-care & mobility will soon be phased out. They will be replaced by individual ERFs for each equipment category.
When will I know about these changes?
All eligible clinicians who have accessed EnableNSW in the last two years will soon receive tailored communication about this change according to the equipment area they prescribe. Please keep an eye out on your emails or visit the EnableNSW website for the most up to date information.
