Forms and funding criteria by program

EnableNSW has funding criteria for each equipment category.

The funding criteria have been developed in consultation with expert clinicians and are designed to specify when equipment will be funded by EnableNSW for each category of assistive technology (AT). They outline what items are provided, the qualifications and experience required to prescribe the AT and provide a basis for consistent and transparent decision making.

The list of items in the funding criteria is not exhaustive and EnableNSW reserves the right to make the final decision regarding the provision of equipment that is not specifically included or excluded in the funding criteria for a particular equipment category.

EnableNSW equipment catalogue

Choosing a product is quick and easy when searching the EnableNSW equipment catalogue. Use this online catalogue to find the product(s) you are looking for and add the SKUID code (this is the products unique code) to the relevant form(s). All products available will have a banner to help identify where the product is coming from once approved, including:

  • stock – these self-care and mobility products are available statewide through the equipment allocation program and are readily available for delivery
    • clearance – these self-care and mobility products are not part of the standard stock equipment range and are one-off items available to people in metropolitan Sydney.
  • contract – these products are available through a NSW Government contract and will be supplied by one of our supplier partners
  • not listed – these products will be purchased from one of our supplier partners and will require a quote
